How to Make a Referral

The Limerick Advocacy Service will accept referrals from:

  • A person with a physical and/or sensory disability living in West Limerick or Limerick City between the ages of 18 - 65.

  • A family member, friend or carer of the above

  • A group (i.e. an advocacy group of people with physical and/or sensory disability)

  • A Service Provider

  • A Public Health Nurse

  • A General Practitioner

  • A Voluntary Body

  • A Whistleblower

Where necessary the advocate will assist with the completion of the referral form. If you cannot open the Advocacy Charter please click on the icon below.

Following the receipt of a referral the advocate will meet with the individual and gather information on the potential advocacy case.

The Limerick Advocacy Service will then make a decision as to whether the service is the most appropriate service to progress the case.
